Fairview Health Services Team Coordinator - Clinic in St Paul, Minnesota

Overview

As a member of the care team the team coordinator performs quality patient- and family-centered care principles through performing a variety of high-level functions within a care team to support the needs of the healthcare team and patients/families. The team coordinator manages workflow by coordinating and maintaining provider schedules, uses a variety of communication tools and systems to handle medical information requests and updates, communicates normal test results either through the direction of the RN or provider or by following established protocols, and works collaboratively with the assigned unit while handling multiple tasks, interruptions, and changes. As a member of the care team, the team coordinator will maintain complete, timely and accurate documentation for each patient and continually look for opportunities to improve processes and workflow

Responsibilities Job Description

  • Manage patient and provider schedules for assigned areas.

  • Complete patient registration and check-in process.

  • Verifies patient demographic/employer and insurance information.

  • Performs telephone message taking and directs communication in a timely and professional fashion.

  • Manage multiple technology applications.

  • Manages MyChart requests including patient sign-up, responding to patient messages and resetting passwords.

  • Multi-task and prioritize appropriately.

  • Continuously improve work processes, environment, and the overall experience to serve customers and meet business goals.

  • Other duties as assigned.

  • Provides patient education based on as assessment of learning needs of patient/care giver.

Qualifications

Required Qualifications

  • Successful completion of formal medical training program such as MA, HUC, NA, or Medical Secretary,

  • OR minimum of 1-year comparable experience in a healthcare settings

Preferred Qualifications

  • 2 or more years’ experience in a medical setting

Additional Requirements :

Excellent public relations, communication, prioritization skills. Proficient telephone and computer skills. Flexible, efficient, ability to embrace change. Knowledge of third-party payors preferred. Knowledge of medical terminology and abbreviations preferred. Demonstrated focus on customer service/patient service with every interaction. Must be flexible to work various shifts and locations as required. Ability to work independently, demonstrate critical thinking skills and engage in ongoing process improvement.
