JOB REQUIREMENTS: The Community Support Specialist (CSS) works with Community Living Alliance (CLA) clients\' to facilitate mental health and/or AODA recovery and support them to achieve their highest level of independent functioning, stability, and community connection. As part of their work CSSs\' conduct client assessments and care plans, coordinate and facilitate services, and support continuity of care upon discharge. Provide necessary support for individuals working toward recovery using a supportive, strengths-based, culturally humble approach to treatment. Facilitate and coordinate services relevant to individuals\' recovery plans and community support needs. Maintain documentation within CLA and county information systems including but not limited to case notes, goal development, and consumer service related activities. Participate & collaborate in client team meetings and service coordination referrals. Utilize effective strategies to engage program participants, enhance motivation, and encourage meaningful, collaborative, and strengths-based goal planning and progress. Plan and facilitate team meetings, conduct resource referrals, assist with coordinating life transition, employment or move planning. Provide screening and assessment, service linkage, service facilitation and other allowable and appropriate services on the CCS service array in a manner that is trauma-informed, culturally relevant, and non-stigmatizing. Assist the client with identifying and establishing natural and professional supports in the community. Maintain contact with client to a degree sufficient to support effective engagement as well as monitor and assist progress. Contribute to program design, policy development and program implementation. ***** OTHER EXPERIENCE AND QUALIFICATIONS: Six (6) months experience working with individuals with mental health and/or substance use conditions, intellectual or physical disabilities, chronic health conditions or seniors, required. Case Management experience, community based mental health services and/or working with people with disabilities, preferred.
