Responsible for the coordination of referrals, including obtaining all necessary intake documents, assisting individuals and families with the completion of paperwork, and assessing individual’s appropriateness for the service to which they are being referred. Handles outreach and coordination with local businesses and agencies for Community Based Pre-Vocational placement sites. Handles outreach, coordination, and scheduling of events and activities for Community Habilitation. Requirements: Associates degree preferred; significant experience in lieu of degree may be considered2-3 years of experience working with individuals with developmental disabilities preferred; at least one year coordinating and scheduling work tasks. Sound decision-making skills and judgement. Good verbal and written communication skills. Organizational skills and attention to detail. Ability to meet deadlines and produce work according to established time frames. Must successfully complete a Background Check process.
